D., 1909, personally appeared before me J. B. Collins,\u00a0C.S.C. in and for the State and county aforesaid, William S. Cox, age 66 years,\u00a0and a resident at Trenton postoffice,  in said county and State, and who, being duly\u00a0sworn, makes the following declaration in order to obtain the pension under the\u00a0provisions of an act entitled, \u201cAn act for the relief of certain Confederate Soldiers,\u00a0Sailors and Widows,\u201d ratified March 8, 1907; that he is the identical William S. Cox\u00a0who enlisted in Co LT, C1 Reg N.C. State Troops on or about the __ day of ___, 1862,\u00a0to service in the armies of the late Confederate States, and that while in said service\u00a0at _________, in the State of _________, on or about the _____ day of ________ 186_,\u00a0he received a wound or wounds, etc.<\/p>\n<p>(Applicant will here state the nature and extent of his wounds and disability, so that a\u00a0proper classification can be made under the new Pension Law passed by the General\u00a0Assembly of 1907.  Read said section of said law carefully, and to accomplish the\u00a0classification therein called for let statement here as to nature and extent of wounds,\u00a0disability, etc., be very full and explicit).<\/p>\n<p>He states that on account of his age together with rheumatism he is three fourths\u00a0disabled to perform manual labor.<\/p>\n<p>He further states that he is, and has been for twelve months immediately preceding\u00a0this Application for Pension, a bona fide resident of North Carolina; that he holds no\u00a0office under the United States, or under any State or county, from which he is receiving\u00a0the sum of three hundred dollars as fees or as salary annually; that he is not worth in his\u00a0own right, or the right of his wife, property at its assessed value for taxation to the\u00a0amount of five hundred dollars ($500), nor has he disposed of property of such value\u00a0by gift or voluntary conveyance since the 11th of March, 1885, and that he is not\u00a0receiving any aid from the State of North Carolina or under any other statute providing\u00a0for the relief of the maimed and blind soldiers of the State.<\/p>\n<p>Sworn and subscribed to before me this 5 day of July, 1909\u00a0(signed) J B Collins  Signature of C. C.\u00a0(signed) W S Cox   Signature of Applicant.<\/p>\n<p>Also personally appeared before Edward Wood, who resides at Kinston postoffice in\u00a0Lenoir county and State, a person whom I know to be respectable and entitled to\u00a0credit, and being by me duly sworn, says he is acquainted with Wm S Cox, the \u00a0applicant for pension, and has every reason to believe that he is the identical person\u00a0he represents himself to be, and that the facts set forth in this affidavit are correct to\u00a0the best of his knowledge and belief, and that he has no interest, direct or indirect, in\u00a0this claim.<\/p>\n<p>Sworn and subscribed to before me this 5 day of July, 1909<\/p>\n<p>(signed) J B Collins  Signature of C. Monk a physician in good standing in\u00a0the said county and State, and being duly sworn, says that he has carefully and\u00a0thoroughly examined WS Cox, the applicant for pension, and finds such disability for\u00a0manual (end of this page JH)<\/p>\n<p>(This page was not filled in except at the bottom where it is signed-JH)<\/p>\n<p>STATE OF NORTH CAROLINA\u00a0_______COUNTY.<\/p>\n<p>To the Auditor of the State of North Carolina<\/p>\n<p>We certify that we have carefully examined the application of ________ for pension\u00a0under the provisions of an act entitled \u201cAn act for the relief of certain Confederate\u00a0Soldiers, Sailors and Widows,\u201d ratified March 8 1907, and the proofs filed in support\u00a0thereof; that we are satisfied the said __________ is the identical person who enlisted\u00a0in Company ________, Regiment of _________ Troops, on or about the ___ day\u00a0of  _____ 186_, and who was disabled in a manner and to the extent stated in the\u00a0foregoing certificates of himself and physician in consequence of a wound received\u00a0in battle on or about the ___day of __________, 186_, and we certify the following\u00a0allegations set forth in his application to be true, namely:  That he is, and has been\u00a0for twelve months immediately preceding this Application for Pension, a bona fide\u00a0resident of North Carolina; that he holds no office under the United States, or under\u00a0any State or county, from which he is receiving the sum of three hundred dollars as\u00a0fees or as salary annually; that he is not worth in his own right, or the right of his\u00a0wife, property at its assessed value for taxation to the amount of five hundred dollars\u00a0($500), nor has he disposed of property of such value by gift or voluntary conveyance\u00a0since the 11th of March, 1885, and that he is not receiving aid from any other source\u00a0from the State of North Carolina, and therefore his application is correct and just under\u00a0the act.<\/p>\n<p>(SEAL) _______________________<\/p>\n<p>Clerk Superior Count.<\/p>\n<p>(IMPRESS COUNTY SEAL HERE).<\/p>\n<p>(DIS is written before the typed word Approved \u2013JH)<\/p>\n<p>Dis-Approved:<\/p>\n<p>(signed) JW Wooten<\/p>\n<p>(signed) TM Dixon  County Pension Board<\/p>\n<p>(signed) W.
