B. Collins, CSC is at the head but it is\u00a0signed by Plato Collins at end)<\/p>\n<p>On this 3rd day of Sept, A. D., 1910, personally appeared before me J.\u00a0B. Collins, C.S.C. in and for the State and county aforesaid, J W\u00a0Burkett, age 82 years, and a resident at Dover, RFD #2 postoffice, in\u00a0said county and State, and who, being duly sworn, makes the following\u00a0declaration in order to obtain the pension under the provisions of an\u00a0act entitled, \u201cAn act for the relief of certain Confederate Soldiers,\u00a0Sailors and Widows,\u201d ratified March 8, 1907; that he is the identical J\u00a0W Burkett who enlisted in Co C, 27 Reg. N.C. State Troops on or about\u00a0the 1st day of April, 1861, to service in the armies of the late\u00a0Confederate States, and that while in said service at Reauns Station,\u00a0in the State of Va, on or about the _____ day of ________ 186_, he\u00a0received a wound or wounds, etc.<\/p>\n<p>(Applicant will here state the nature and extent of his wounds and\u00a0disability, so that a proper classification can be made under the new\u00a0Pension Law passed by the General Assembly of 1907.  Read said section\u00a0of said law carefully, and to accomplish the classification therein\u00a0called for let statement here as to nature and extent of wounds,\u00a0disability, etc., be very full and explicit).<\/p>\n<p>Wounded over left eye \u2013 never been able to see much of that eye and now\u00a0the other one is affected.\u00a0He further states that he is, and has been for twelve months\u00a0immediately preceding this Application for Pension, a bona fide\u00a0resident of North Carolina; that he holds no office under the United\u00a0States, or under any State or county, from which he is receiving the\u00a0sum of three hundred dollars as fees or as salary annually; that he is\u00a0not worth in his own right, or the right of his wife, property at its\u00a0assessed value for taxation to the amount of five hundred dollars\u00a0($500), nor has he disposed of property of such value by gift or\u00a0voluntary conveyance since the 11th of March, 1885, and that he is not\u00a0receiving any aid from the State of North Carolina or under any other\u00a0statute providing for the relief of the maimed and blind soldiers of\u00a0the State.<\/p>\n<p>Sworn and subscribed to before me this 3 day of Sept, 1910<\/p>\n<p>(signed) Plato Collins  Signature of C. Rayner , who resides at Kinston\u00a0postoffice in said county and State, a person whom I know to be\u00a0respectable and entitled to credit, and being by me duly sworn, says he\u00a0is acquainted with J. W. Burkett, the applicant for pension, and has\u00a0every reason to believe that he is the identical person he represents\u00a0himself to be, and that the facts set forth in this affidavit are\u00a0correct to the best of his knowledge and belief, and that he has no\u00a0interest, direct or indirect, in this claim.<\/p>\n<p>Sworn and subscribed to before me this 3rd day of Sept, 1910<\/p>\n<p>(signed) Plato Collins  Signature of C. Pollock a physician in good\u00a0standing in the said county and State, and being duly sworn, says that\u00a0he has carefully and thoroughly examined J. W. Burkett, the applicant\u00a0for pension, and finds such disability for manual (end of this page JH)<\/p>\n<p>STATE OF NORTH CAROLINA,\u00a0Jones COUNTY.<\/p>\n<p>To the Auditor of the State of North Carolina.\u00a0We certify that we have carefully examined the application (this is\u00a0written in JH) to increase pension of JW Burkett for pension under the\u00a0provisions of an act entitled \u201cAn act for the relief of certain\u00a0Confederate Soldiers, Sailors and Widows,\u201d ratified March 8 1907, and\u00a0the proofs filed in support thereof; that we are satisfied the said JW\u00a0Burkett is the identical person who enlisted in Company C, 27 Regiment\u00a0of NC Troops, on or about the 1st day of April 1861, and who was\u00a0disabled in a manner and to the extent stated in the foregoing\u00a0certificates of himself and physician in consequence of a wound\u00a0received in battle on or about the ___day of __________, 186_, and we\u00a0certify the following allegations set forth in his application to be\u00a0true, namely:  That he is, and has been for twelve months immediately\u00a0preceding this Application for Pension, a bona fide resident of North\u00a0Carolina; that he holds no office under the United States, or under any\u00a0State or county, from which he is receiving the sum of three hundred\u00a0dollars as fees or as salary annually; that he is not worth in his own\u00a0right, or the right of his wife, property at its assessed value for\u00a0taxation to the amount of five hundred dollars ($500), nor has he\u00a0disposed of property of such value by gift or voluntary conveyance\u00a0since the 11th of March, 1885, and that he is not receiving aid from\u00a0any other source from the State of North Carolina, and therefore his\u00a0application is correct and just under the act.<\/p>\n<p>(SEAL) (signed) JB Collins<\/p>\n<p>Clerk Superior Count.<\/p>\n<p>(IMPRESS COUNTY SEAL HERE).<\/p>\n<p>Approved:<\/p>\n<p>(signed) W.
