D., 1896, personally appeared before me S. E. Koonce,\u00a0C.S.C. in and for the State and county aforesaid, Mrs. C. E. Chase, age 55 years,\u00a0and a resident at Comfort post office, in said county and State, and who, being duly\u00a0sworn, makes the following declaration in order to obtain the pension under the\u00a0provisions of an act entitled, \u201cAn act for the relief of certain soldiers of the late war\u00a0between the States,\u201d  ratified March 11, 1889; as amended by acts of General\u00a0Assembly of 1891, 1893 and 1895; that she is the widow of the late Wm. H Chase,\u00a0who enlisted in the Co B, 51st  Reg. N.C. State Troops on or about the 21st day of\u00a0March, 1862, to serve in the armies of the late Confederate States, and that while in\u00a0performance of duty in said Company and Regiment, in the State of ______, on or\u00a0about the ____day of ________ 186_, he received a wound or wounds or injuries\u00a0which terminated his life.<\/p>\n<p>(Applicant will here state very particularly when and where the husband died;\u00a0whether he was in the discharge of his duty as a soldier or sailor at the time of the\u00a0receipt of the wound or injury or contracting of the disease; whether the death was the\u00a0probable consequence of such service and all other circumstances necessary to\u00a0establish the fact of service and death of husband, etc.)<\/p>\n<p>W.H. Chase husband of this applicant died November 6th, 1895.\u00a0She further states that she has never re-married; that she is, and has been for twelve\u00a0months immediately preceding this Application for Pension, a bona fide resident of\u00a0North Carolina; that she holds no office under the United States, or under any State\u00a0or county, from which she is receiving the sum of three hundred dollars as fees or as\u00a0salary annually; that she is not worth in her own right, or the right of her late husband,\u00a0property at its assessed value for taxation to the amount of five hundred dollars ($500), nor has she disposed of property of such value by gift or voluntary conveyance since\u00a0the 11th of March, 1885.<\/p>\n<p>Sworn and subscribed to before me this ___day of _______, 189__<\/p>\n<p>_____________ Signature of C. Chase who resides at Comfort Post office, in said county and State,\u00a0physician\u2026in good standing whom I know to be respectable and entitled to credit,\u00a0and being by me duly sworn, say that they are acquainted with __________ , the\u00a0widow of the late _________, of Co____, ____ Regiment North Carolina State Troops,\u00a0and that they believe her to be the identical person she represents herself to be, and\u00a0that the facts set forth in her affidavit are correct to the best of their knowledge and\u00a0belief, and that they have no interest, direct or indirect, in this claim.<\/p>\n<p>Sworn and subscribed to before me this __day of ______.189_<\/p>\n<p>____________ Signature of C.
